Graduate Student Forms
U.S. students need to fill out and mail back all the below forms, as well as the W-4 included with your welcome letter, to Chemical Engineering by the deadline given in the letter.
Note: Please use your permanent address on all forms if you do not yet have a local address.
Foreign students will need to fill out the forms below upon arrival. Please see Katie Shearer (rm. 238 Colburn Lab) immediately when you get to Delaware.

- Include a voided check or savings/deposit slip with this form. If you have new checks please make sure to write your name and address at the top.
- You may use your current bank or establish a local bank. If you want to begin with your current bank and change to a local bank later this is also an option.
- Be sure to sign the form.

Fill out Section 1, sign, and include a photocopy of either your passport OR, your driver’s license AND social security card. (see list of acceptable documents pg. 3 of form)
